Disabled persons must have either a current Florida driver's license or a Florida identification card issued by the Florida Department of Highway Safety and Motor Vehicles. An exemption is issued by the certifying authority if the disabled person is physically unable to visit or be transported to a driver's license office to obtain this documentation.
All disabled persons must submit the Application for Disabled Person Parking Permit (Form HSMV 83039) .
Permanent parking permits are issued for a period of up to four years and expire on the applicant’s birthday. Temporary parking permits are issued for up to six months, as specified by the certifying authority.
Subsequent or additional permanent parking permits may be issued to individuals who are certified as quadriplegic or who are frequent travelers, as certified on Form HSMV 83039 .
Organizations that provide regular transportation services to disabled persons must provide either a Federal Employer Identification Number (FEID) or a Florida sales tax registration number to be printed on the disabled parking permit.
Organizations may be issued as many parking permits as they have vehicles providing transportation to disabled persons.
The disabled person parking permit must be hung from the rearview mirror of the vehicle used to transport a disabled person when parked in a designated disabled person parking space.
Please note that the disabled parking permit is associated with the driver’s license or identification card, not with a specific vehicle. The permit may be used in any vehicle as long as the permit holder is present.
Disabled person parking permits are given to people who are medically certified to have long-term, permanent or temporary mobility impairment, or who are certified legally blind or temporarily sight-impaired.
